Record savings results highlight benefits of value-based care for patients, primary care and Medicare
BETHESDA, Md. – Aledade, the nation’s largest network of independent primary care practices, announced today that its accountable care organizations (ACOs) saved Medicare more than $1 billion in 2024 through the Medicare Shared Savings Program (MSSP). These savings are the result of improving the quality of care and outcomes for more than 1 million Medicare beneficiaries. Aledade ACOs cared for approximately 11% of all MSSP beneficiaries and generated nearly 20% of the program’s shared savings. In fact, Aledade’s most experienced ACOs – representing a quarter of its beneficiaries – achieved over 10% savings, compared to just 5% for beneficiaries in non-Aledade ACOs.
The Centers for Medicare & Medicaid Services (CMS) recently released Performance Year 2024 results for MSSP, reporting that Aledade ACOs:
"These results prove that value-based care works – for patients, clinicians and the Medicare program,” said Farzad Mostashari, M.D., co-founder and CEO of Aledade. "By supporting primary care clinicians in their transition to value-based care, we are improving patient lives, strengthening primary care and saving taxpayer dollars."
Value-Based Care Outperforms Fee-for-Service
Aledade’s network, composed of primary care practices and community health centers across the nation in rural, suburban and urban settings, demonstrated that paying for value instead of volume is better for primary care and patients. A remarkable 93% of Aledade ACOs earned shared savings, compared to 73% of non-Aledade ACOs across the entire MSSP. Aledade’s ACOs earned approximately $390,000 in shared savings per practice.
Community Health Center Success
Aledade’s effectiveness across various geographies and patient populations is further highlighted by the fact that its network includes more than 25% of all community health centers (CHCs) in MSSP. These organizations face unique challenges serving beneficiaries in underserved communities, requiring additional dedicated resources and innovative approaches to help them reach this level of success. Better Care, Better Outcomes
Aledade partners provided proactive and preventive primary care, managed chronic conditions and reduced costly hospitalizations. Aledade’s most experienced ACOs – representing a quarter of its beneficiaries – now routinely achieve double the benchmark savings.

Across its ACO network, several Aledade partners earned high rankings across various patient satisfaction metrics, including timely care, communication and access to specialists. Aledade’s highest-performing ACO, the Louisiana ACO, which started in 2016, ranked No.1 in timely appointments, care coordination and provider communication. Their exceptional efforts generated savings of more than 17%.
During the 2024 MSSP Performance Year, Aledade partnered with over 1,900 primary care organizations across 59 ACOs that serve more than 1 million Medicare beneficiaries.
